摘要: 本研究目的在於比較台中地區牧場及市區野鼠內寄生蟲相之差異及Capillaria hepatica感染之野鼠肝臟組織病理學與Gelatinase表現。自2004年6月至2005年12月,於台中縣轄內牧場、台中市傳統市場及中興大學校區內共捕獲133隻野鼠,計為溝鼠109隻、屋頂鼠14隻及錢鼠10隻,予以麻醉犧牲,取其內臟(心臟、肺臟、肝臟、胃、小腸、大腸及膀胱)、肌肉及糞便進行檢查。結果台中縣牧場之野鼠內寄生蟲盛行率為93.7%,而台中市盛行率為94.7%,總陽性盛行率為94.0%。寄生蟲盛行率最高為錢鼠100%,其次為溝鼠95.4%及屋頂鼠78.6%。依寄生蟲種類區分為11種線蟲、4種絛蟲及Sarcocystis屬原蟲,線蟲之盛行率為94.0%,其中包括Strongyloides ratti 83.5%、Nippostrongylus brasiliensis 59.4%、C. hepatica 48.1%、Trichosomoides crassicauda 40.4%、Heterakis spumosa 28.6%、Angiostrongylus cantonensis 15.0%、Capillaria sp. 20.5%、Syphacia muris 2.3%、Gongylonema neoplasticum 0.8%、Trichurus sp. 0.8%及Physaloptera sp. 0.8%,絛蟲為73.7%其中包括Cysticercus fasciolaris 43.6%、Raillietina celebensis 34.6%、Hymenolepis diminuta 29.3%及H. nana 10.5%,原蟲為Sarcocystis spp. 28.7%。而Sarcocystis spp.及Physaloptera sp.為台灣野鼠寄生蟲調查中首次報告,此外共檢出6種人畜共通寄生蟲為C. hepatica、A. cantonensis、R. celebensis、H. nana、H. diminuta及Sarcocystis spp.,以C. hepatica之盛行率最高。將感染C. hepatica之肝臟經H&E及Sirius red染色可見蟲體、壞死蟲體及蟲卵引起肝臟局部壞死及炎症反應,並可見其為纖維結締組織所圍繞及包被,程度不一,並可見瀰漫性之纖維化病變,病變區之Gelatinase活性分析,得知纖維化之肝臟其proMMP-9 (92 kDa)表現明顯高於正常之肝臟組織。C. hepatica所造成之病變為肝臟纖維化,而目前台中地區C. hepatica於野鼠盛行率高,尤其是溝鼠,且溝鼠棲息地又與人類息息相關,因此人類受感染的問題值得重視。
The aim of this study is to investigate the prevalence of endoparasites and to study of histopathology and gelatinase expression on liver infected by Capillaria hepatica in wild rats in Taichung. A total of 133 wild rats (109 Rattus norvegicus, 14 R. rattus and 10 Suncus murinus) trapped in Taichung county farms, Taichung city markets and NCHU, were slaughtered for the survey of endoparasites from June 2004 to December 2005. The visceral organs (heart, lung, liver, entire digestive tract and urinary bladder), diaphragm and feces were collected and examined for helminthes. The rats were found to be positive for parasites was 93.7% in Taichung county, and 94.7% in Taichung city. The infection rate was 94.0% (95.4% in R. norvegicus, 78.6% in R. rattus and 100% in S. murinus). Furthermore, 11 nematodes (Angiostrongylus cantonensis 15.0%, Capillaria hepatica 48.1%, Capillaria sp. 20.5%, Nippostrongylus brasiliensis 59.4%, Strongyloides ratti 83.5%, Heterakis spumosa 28.6%, Syphacia muris 2.3%, Trichosomoides crassicauda 40.4%, 1 case of Trichurus sp., Gongylonema neoplasticum and Physaloptera sp.), 4 cestodes (Cysticercus fasciolaris 43.6%, Hymenolepis diminuta 29.3%, H. nana 10.5% and Raillietina celebensis 34.6%), and 1 protozoans (Sarcocystic spp. 28.7%). Sarcocystic spp. and Physaloptera sp. were first recorded in this study in Taiwan. Among them, 6 zoonotic parasites were identified such as C. hepatica (48.1%), A. cantonensis (15.0%), R. celebensis (34.6%), H. diminuta (29.3%), H. nana (10.5%) and Sarcocystis spp. (28.7%). Liver of rats infected by C. hepatica were collected on the study of histopathology and gelatinase activity. Scattered focal lesions were shown in histological section stained with H&E and Sirius red for collagen. Dead worms and their eggs induced focal necrosis and inflammation within the liver, which were surrounded by fibrous capsule. Fibrosis was diffused in all infected livers. Analyzing gelatinase activity of the lesions indicated the increased expression of proMMP-9. It has been reported C. hepatica can induce hepatic septal fibrosis in humans and other mammals. The higher ratio of C. hepatica was noted in this study, indicating the importance of prevention on this parasite which may transfer to humans from the wild rats infected by C. hepatica.
