dc.description.abstractIFN-γ是肺結核和敗血症患者在感染初期所參與免疫反應的細胞激素之一,當被病原菌感染時,可能會因為缺乏IFN-γ而導致死亡,這表示了IFN-γ在生物體內的免疫反應中扮演一個重要的角色。許多文獻指出IFN-γ與肺結核和敗血症的易感性相關,但對於IFN-γ的免疫機轉尚未清楚了解。肺結核病主要是引發細胞免疫反應,當感染分枝桿菌後會活化CD4+ T細胞,分泌IFN-γ並活化巨噬細胞進行吞噬作用;敗血症是前發炎物質及抗發炎物質同時表達而形成複雜的綜合症,當兩種物質無法在體內維持平衡時,則會引起多器官功能衰竭。本研究主要目的是在探討肺結核和敗血症患者中IFN-γ mRNA的表現量和基因的多型性與疾病的易感性。 本實驗從肺結核和敗血症患者的血液中取得PBMCs,並從PBMCs中萃取出genomic DNA和mRNA,genomic DNA部份進行nested PCR得到文獻中經常發生基因多型性區域的IFN-γ基因片段,長約有1800bp,包含promoter、exon1、intron A和部份exon2,將其解出序列後與Database基因庫中的IFN-γ genomic DNA序列做比對,探討台灣人是否也有單一核苷酸多型性的現象發生。mRNA部份進行反轉錄合成cDNA,經由PCR的技術得到長約540bp的cDNA IFN-γ PCR產物後跑電泳,藉用濃度測定軟體觀測PCR產物的濃度,並以SPSS統計軟體進行數據處理分析。 研究結果發現,肺結核和敗血症患者與正常人的IFN-γ -179G/T promoter (負號表示從transcription site計算)位置上沒有發現基因多型性的存在。肺結核患者的IFN-γ mRNA的表現量與正常人並無明顯的差異,但是在敗血症患者中卻比正常人明顯來的少。另外, IFN-γ和IL-4之間的平衡與肺結核和敗血症的關係在本研究中尚未明確,未來還需要再更深入探討。zh_TW
